Daily Rest
The gap between finishing and starting again is the limit most often breached and least often monitored, because no report in most organisations computes it.

An uninterrupted weekly rest sounds simple until it may be averaged across a fortnight, at which point twelve consecutive days becomes compliant.

The Swap That Destroys the Gap
Two people agree to exchange shifts, the supervisor nods, and a rest period that the rota guaranteed quietly disappears from a week nobody will ever check.
Call-Outs at Night
A forty-minute call at two in the morning does more damage to a rest period than its length suggests, and the system records only the forty minutes.
Compensatory Rest
Most regimes permit rest to be reduced provided equivalent rest follows. The permission is widely used and the condition almost never applied.
What the Rota Tool Does Not Check
Scheduling software checks what it was configured to check, and the rest rules were left at their defaults by somebody working to a deadline.
